Chlorhexidine helps control plaque and eases localised gingival and oral infections, small amounts of fluoride ions present are taken up by tooth structure and help to increase resistance to dental caries improves the binding of chlorhexidine onto tooth surfaces. Carbamide Peroxide 10% gel is a plaque inhibitor that is particularly effective against gram –ve bacteria and has a demonstrated ability to bleach tooth structure with varying degrees of effectiveness. By alternating daily applications of chlorhexidine and carbamide peroxide bacterial plaque is inhibited, particularly gram –ve. The carbamide peroxide inhibits chlorhexidine staining and the fluoride in the Chlorofluor reduces cervical sensitivity caused by the carbamide peroxide.
Indications
To assist with the control of chronic gingival inflammation periodontal disease and caries gingivitis, periodontal disease, peri implantitis and dental caries. The product may be used to reconstitute oral plaque prior to extensive dental treatment and for bleaching teeth for patients who have exhibit cervical sensitivity to Carbamide Peroxide.
Contraindications
Lichen Planus may be exacerbated by carbamide peroxide gel and treatment is contraindicated for these patients. carbamide peroxide will remove the smear layer from dentine but has no reported deleterious effects on tooth structure. Children under 3 years of age and those unable to expectorate the gels.
Precautions
Isolated cases of uneven bleaching or banding may occur, and patients must be advised of this rare possibility. Sore throats from swallowing gel leaching out from poorly fitting trays have been reported. As with most medication, use during pregnancy is not advised. The trays should not be worn during eating, teeth cleaning, playing contact sport or swimming and should be stored in the container provided whilst out of the mouth. Avoid contact with eyes. The use of chlorhexidine alone as a mouthwash may cause a harmless superficial staining on teeth.
Directions for Use
Initial Visit: Involves the recording of pocket depths, mobility, gingival colour and taking an alginate impression of both arches. Once a plaster model has been fabricated from the impression, the soft thermoplastic blanks provided may be used to construct the trays. This should be done using a commercially available apparatus with a high temperature element and vacuum motor. A close fitting tray that does not extend extends just beyond the cervical margins is essential for a successful result. The individual’s patient pack is then prepared for handover.
Second Visit: Involves the fitting and checking of the thermoplastic trays. The patient should also be instructed on precautions and indications on use of the Perio Pack system.
Subsequent Appointments: The patient should be recalled in a month to review progress, observe and record changes to gingival and periodontal tissues. Continue to recall patient as often as required until desired the outcome is achieved. The Perio Pack system may be used as an adjunct to standard periodontal treatment for extended periods.
Poisoning
If poisoning occurs contact a doctor or Poisons Information Centre. If swallowed, do not induce vomiting. Give a glass of water. If skin contact occurs, remove contaminated clothing and wash skin thoroughly. If in eyes, hold eyes open, flood with water for at least 15 minutes and seek medical assistance.
